New Patient & Insurance Information

New Patients will need to bring a copy of their insurance cards and photo identification to their appointments. On your first visit, please arrive 15-20 minutes prior to your appointment time to verify your payment and insurance information. We will contact you to schedule your first evaluation and treatment.

INSURANCE INFORMATION

REFERRALS

A physician’s referral is needed in order to file insurance claims for services. This can be faxed directly to us from your physician’s office. Your physician’s office or you can also contact us to start the referral process today! We will contact you to schedule the initial evaluation.

Health insurance policies are a lot to navigate, and plans/policies do differ. Pinnacle Therapies will work with you and your insurance company prior to your first visit to obtain benefit information as a courtesy. Pinnacle Therapies recommends that you also contact your insurance provider directly prior to your initial visit to find out what type of coverage your policy provides and whether your specific situation/concerns will be covered for evaluation and services. It is the patient’s responsibility to understand what types of coverage is provided by their health insurance policy and if they meet the requirements specified in the plan. Your insurance insurance company will inform you of -if any- co-pays, deductibles, and your level of coverage specific to your policy. Some other information that is very useful to know about your policy before scheduling an appointment with us are questions like:

  • Does my policy cover both an assessment and treatment?
  • Does my policy provide coverage for my dependents to receive an evaluation and treatment?
  • How many evaluations/assessments does my policy allow yearly?
  • How many visits/sessions does my policy allow yearly?
  • Are there any exclusions or limitations to therapy services?
  • Is coverage limited to conditions that are acquired or rehabilitative or exclude coverage for services considered “developmental” or “educational”?
  • Does my plan cover receiving services from an out-of-network provider?
  • Do I need prior authorization or precertification? (provider or yourself will need to call in advance to receive approval prior to starting therapy services)

Some policies do have limitations or exclusions, which are services that could be required for treatment but not covered by your policy. Your insurance company will explain these limitations and exclusions. It is your responsibility to know these limitations. We will do our best to work with your specific policy, but an insurance policy is a contract between you and the insurance company. Any services billed that are not covered by your policy are your responsibility. It is important to first review your specific health insurance coverage and limitations prior to receiving services.

Pinnacle Therapies accepts three payment options. In-Network, Out-of-network, and Private Pay. This allows us to ensure accessibility to appropriate healthcare services for all.

We are currently In-Network with the following insurance companies:

  • Medicaid
  • Medicare
